This presentation delves into some of the most common human error traps, such as inaccurate assumptions, limited working memory, miscommunication, and complacency. It outlines how Human Performance Improvement (HPI) tools can be utilized to prevent errors and enhance resilience. The tools discussed include task preview, validating assumptions, maintaining a questioning attitude, and conducting both self and peer checks. The presentation also features the analysis of two case studies from an HPI perspective, identifying error traps and determining which tools could have been employed to prevent the incidents. Additionally, the presenter shares valuable insights and experiences from his extensive career in the health and safety field, engaging the audience with thought-provoking questions to foster learning and reflection.
