About me
After graduating from Texas A&M University with a BS in Aerospace Engineering in 1969, Bob was employed by McDonnell Douglas in St. Louis, MO as an Aeronautical Loads Engineer from 1970 to 1974 and worked on the F-4 Phantom. In 1974, he was hired by Allied Chemical Corporation as a Reliability Engineer, eventually becoming their chief "failure investigator, " which took him all over the USA and Canada. In 1985, Bob formed Failsafe Network, Inc. to "help people learn from things that went wrong." His "Latent Cause Analysis" process has been promoted and used world-wide. Bob was a member of the HPRCT board from 2016 to 2019. After passing his business to his long-time associate, Rob Statham, he eventually retired in 2020. He currently acts as an occasional advisor to Mr. Statham.
