Mr. Wreathall is a specialist in systems-engineering methods with particular emphasis on probabilistic risk assessment methods and human and organizational performance as they relate to safety, reliability and quality. He has led or participated in six full-scale risk assessments, including those for four nuclear power plants, one chemical process plant and one assessing risks in heart transplantation. He has pioneered the development and application of human-performance analysis methods, both quantitative and qualitative, for application in the chemical engineering, transportation, nuclear, and medical communities.
He has participated in the development of the understanding of human errors and the circumstances that lead to their occurrence, including presenting at international conferences on the subject sponsored by NATO, the World Bank, the US Nuclear Regulatory Commission, and the International Atomic Energy Agency. He has been a member of the Technical Advisory Panel for the National Quality Forum’s Committee to develop taxonomies for reporting human errors in healthcare.
He was an invited participant to the First International Symposium on Resilience Engineering, in Soderköping, Sweden, and the 2005 International Seminar on Resilience Engineering and Cognitive Systems, Rio de Janeiro, Brazil. He was the invited keynote speaker at the first South American Conference on Safety and Security in Work and its Environment, Porto Alegre, Brazil, in 2004. He has performed numerous projects for clients in Asia and Europe.
